Abstract The two-level fuzzy-lattice (2L-FL) learning scheme is introduced for application on an intelligent surgical (mechatronic) drill in the stapedotomy surgical procedure in the ear. The 2L-FL scheme learned from past cases to evaluate pointedly the thickness of a stapes bone using a force/torque pair of drilling profiles. Hence it is possible, in principle, to drill safely a hole through supple stapes by retracting automatically the drill upon bone breakthrough. The 2L-FL scheme was applied on two different partly ordered sets defined on a set of square matrices mapping features of two different data profiles to one another. Results are presented comparatively on experimental drilling data. We also discuss extension of our techniques to other surgical and industrial applications especially ones involving fusion of disparate sensory data, where effective learning could compensate for the lack of accurate analytical models.

Can the learning curve in stapes surgery predict future functional outcome?

Over the last 20 years, the number of stapes operations performed has decreased steadily. This inadequate exposure to stapes surgery poses problems for both trainees and trainers. We retrospectively reviewed the outcomes of stapedotomy performed by a young physician at the ENT Clinic of the "A. Gemelli" Hospital of Rome. We used the technique of "one-shot" CO2 Laser stapedotomy using a titanium...

The Profile of Otoacoustic Emissions and Multifrequency Tympanometry in Otosclerotic Patients Undergoing Two Types of Stapes Surgery: Small Fenestra and Microtraumatic Stapedotomy

BACKGROUND Otoacoustic emissions (OAEs) are influenced in otosclerosis. The aim of the current study was to investigate the profile of transient evoked (TEOAEs) and distortion product otoacoustic emissions (DPOAEs) in association with multifrequency tympanometry measures in otosclerotic patients undergoing 2 types of stapes surgery: small fenestra and microtraumatic stapedotomy. MATERIAL AND ...

Stapedotomy in a residency training program.

INTRODUCTION The major objective of a stapedotomy is the restoration of hearing. In training centers, the hearing results obtained by resident physicians are lower than those achieved by more experienced surgeons. An analysis of the surgical results obtained by resident physicians is essential to identification of the deficiencies in the learning process. OBJECTIVE To analyze the postoperativ...

Biomechanical Evaluation of Temperature Rising and Applied Force in Controlled Cortical Bone Drilling: An Animal in Vitro Study

Background: The present study was conducted to quantify the relationships between bone drilling process parameters(i.e., feed rate, resting time, exit rate, and drill bit diameter) and drilling outcome parameters (i.e., thrust force andmaximum temperature). Methods: This study utilized 10-cm cortical bovine samples to evaluate the effects of four independent parameters,in...

Laser Versus Drill-Assisted Stapedotomy for the Treatment of Otosclerosis: A Randomized-Controlled Trial

laseror microdrill-assisted technique may enable better outcomes in terms of intraand post-operative complications as well as of hearing gain as compared with classical stapedotomy technique. Materials and Methods: Eighty-two subjects undergoing primary stapes surgery for otosclerosis were enrolled for this study as having a least pure tone average (PTA) air-bone gap (ABG) of 35 dB. Patients we...
